  • An increase in the number of participants achieving post-graduation education goals; and
  • An increase in the number of participants reaching various components of independent living goals.

Project Description

This project will collaborate with a variety of partners to develop “Empowerment Chapters” designed to improve the transition “flow” and enhance the chances for successful outcomes. Empowerment chapters will build a support team directed by the student and his/her family; and provide information that brings power of choice, self-determination, and self-advocacy. Social networks and mentorships will be constructed to deepen, broaden, and enhance the elements of transition currently in place; creating an environment that reinforces esteem, inclusion, and respect in the broader school community.
