Project Goals

  • An increase in the percentage of youth (who had IEP’s and are no longer in secondary school) that have been competitively employed, enrolled in some type of post-secondary school, or both within one year of leaving high school;
  • An increase in the percentage of youth age 16 and above with an IEP that includes coordinated, measurable, annual IEP goals and transition services that will reasonably enable the student to meet the post-secondary goals;
  • An increase in the percentage of youth with an IEP who graduate from high school with a regular diploma; and
  • A decrease in the percentage of youth with an IEP who drop out of high school.

Project Description

The IAM DETERMINED project is to enable individuals with disabilities to “get a life” after high school. The purpose of the project is to enhance the equality of transition services for students, ages 14-21, which have an intellectual or developmental disability with Individual Education Program (IEP) within the Commonwealth of Kentucky.

The IAM DETERMINED project addresses the critical shortage of interagency collaboration for students with disabilities within the Commonwealth. The project utilizes a team approach to form an Interagency Transition Team consisting of seven team members. Team members include representatives from The Arc of Kentucky, Kentucky Department of Education, Department for Vocational Rehabilitation, Cabinet for Families and Children, Post Secondary Education, Protection and Advocacy, a parent, and a student. The team works to holistically scan and interpret the current transitional program and process within the Commonwealth. In addition, resource mapping is used as a tool for determining the available resources and current procedures of the transitional program for individuals transitioning to adulthood. The results are being used to develop a resource guide specific to regions throughout the Commonwealth; and will be available online at The Arc and Kentucky Department of Education (KDE) websites, in print, and on CD.
