- Students will lead his/her planning process and related activities (IEP, etc.);
- Students will participate in a self-advocacy group or activities;
- Student will have the support they need to develop friendships and participate in activities with family members; and
- Students, who have career awareness in their plan, actually receive it in a way that is consistent with their plan.
The Arc of Massachusetts and The Arc of Greater Haverhill and Newburyport are partnering on a three-year demonstration project focused on intensive transition planning for students in Metro Boston and Newburyport in the Commonwealth of Massachusetts. The project focuses on concrete outcomes for students to be prepared for adult life in the community. These include employment, social and recreational activities, transportation, and housing. A “whole person-systems” approach helps staff focus on individual student goals, skills, and training needs. Students are encouraged to take on a leadership role. Family members are trained to assist the student to develop collaborative relationships for the purpose of achieving the student’s vision.
Components of the project include personal meetings related to students’ individual educational plans and other planning activities; group meetings and training sessions; use of a support broker on planning and implementing transition; interagency outreach and planning; webinars and group meetings; as well as linkage with local businesses and local agencies to implement transition.
